{"id":92419,"date":"2022-08-03T16:38:49","date_gmt":"2022-08-03T14:38:49","guid":{"rendered":"https:\/\/42krunning.com\/iliotibial-band-syndrome-a-common-injury-in-runners\/"},"modified":"2026-08-19T16:48:36","modified_gmt":"2026-08-19T14:48:36","slug":"iliotibial-band-syndrome-a-common-injury-in-runners","status":"publish","type":"post","link":"https:\/\/42krunning.com\/en\/iliotibial-band-syndrome-a-common-injury-in-runners\/","title":{"rendered":"Iliotibial band syndrome, a common injury in runners"},"content":{"rendered":"&#13;\n<p class=\"wp-block-paragraph\">In this post, we offer information about <strong>iliotibial band syndrome<\/strong> or IT band (ITBS), also commonly known as <strong><em>&#8220;<strong>runner&#8217;s<\/strong> band&#8221; <\/em><\/strong>or<strong><em> &#8220;runner&#8217;s knee&#8221;<\/em><\/strong>.<\/p>&#13;\n<p>This is a <strong>very common injury in runners and cyclists<\/strong>, although other athletes also suffer from it. Iliotibial band syndrome causes <strong>severe pain<\/strong> <strong>on the outside of the knee<\/strong> that manifests during or after exercise. <\/p>&#13;\n<p>The pain is caused by the continuous <strong>friction of the iliotibial band<\/strong> \u2013 which runs from the hip to the back of the knee, along the outside of the thigh \u2013 <strong>with the lateral femoral condyle<\/strong> when flexing and extending the leg, leading to irritation and subsequent inflammation.<\/p>&#13;\n<p>It is a <strong>very annoying and limiting injury<\/strong> that can affect athletes of any age and physical condition. For this reason, there is a disparity in how it is approached, as well as a variety of results obtained with currently available treatments. <\/p>&#13;\n&#13;\n<p class=\"wp-block-paragraph\">As we have already mentioned, this is an injury usually associated with runners and is the <strong>main cause of lateral knee pain<\/strong> in these athletes (1). But this fact should not be misleading, as it is an injury that <strong>can also occur in cyclists,<\/strong> accounting for up to 15% of knee injuries in cycling (2). In short, it is a very common injury in our society, representing <strong>22% of lower limb injuries<\/strong> (1, 2).  <\/p>&#13;\n&#13;\n<p class=\"wp-block-paragraph\">Now, you&#8217;re probably wondering what the iliotibial band really is and, above all, why it causes so much trouble for athletes who cover many kilometers running or cycling.<\/p>&#13;\n&#13;\n<h3 class=\"wp-block-heading\">The iliotibial band is not a muscle<\/h3>&#13;\n&#13;\n<p class=\"wp-block-paragraph\">The iliotibial band is a structure formed by <strong>connective tissue<\/strong>. As its name suggests, this tissue is responsible for connecting and supporting structures; that is, it is non-contractile tissue. <\/p>&#13;\n<p><strong>It is not a muscle<\/strong>; it is a tissue with a morphology <strong>more similar to a ligament<\/strong>, without being one. In the case of iliotibial band syndrome, the injury is caused by the recurrent friction of the iliotibial band with the lateral femoral condyle. When the knee flexes between 20\u00ba and 30\u00ba, the iliotibial band slides from anterior to posterior, thus causing the injury mechanism (3).  <\/p>&#13;\n<figure id=\"attachment_49536\" aria-describedby=\"caption-attachment-49536\" style=\"width: 1024px\" class=\"wp-caption aligncenter\"><a href=\"https:\/\/42krunning.com\/wp-content\/uploads\/2022\/08\/camiseta-tecnica-42k-ozone-daylight-pareja-00063.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-49536 size-large\" src=\"https:\/\/42krunning.com\/wp-content\/uploads\/2022\/08\/camiseta-tecnica-42k-ozone-daylight-pareja-00063-1024x1024.jpg\" alt=\"El origen de la cintilla iliotibial, un problema en la articulaci&#xF3;n de la cadera.\" width=\"1024\" height=\"1024\"\/><\/a><figcaption id=\"caption-attachment-49536\" class=\"wp-caption-text\">Es una lesi\u00f3n que puede afectar a deportistas de cualquier edad y condici\u00f3n f\u00edsica.<\/figcaption><\/figure>&#13;\n&#13;\n<p class=\"wp-block-paragraph\"> <\/p>&#13;\n<p>Now, why does this recurrent friction of the iliotibial band occur?<\/p>&#13;\n&#13;\n<h3 class=\"wp-block-heading\">A hip joint problem, the origin of iliotibial band syndrome<\/h3>&#13;\n&#13;\n<p class=\"wp-block-paragraph\">First, it should be noted that multiple studies confirm the argument that <strong>there is no clear cause<\/strong> for the injury. Even so, they all agree on one thing: it is <strong>a problem in the hip joint<\/strong>, either due to a deficit in its range of motion, amount of movement, or a problem at the stabilization level, lack or excess of strength in the muscles responsible for it. <\/p>&#13;\n&#13;\n<p class=\"wp-block-paragraph\">It is also related to an increase in internal rotation of the affected knee (4, 5, 6). In some cases, a reduction in the coordination of the <strong>hip abductor musculature<\/strong> is evident in patients with iliotibial band syndrome; this lack of coordination is associated with a decrease in motor control over these structures, which can also lead to injury (5). <\/p>&#13;\n<figure id=\"attachment_49535\" aria-describedby=\"caption-attachment-49535\" style=\"width: 1024px\" class=\"wp-caption aligncenter\"><a href=\"https:\/\/42krunning.com\/wp-content\/uploads\/2022\/08\/camiseta-tecnica-42k-ozone-eclipse-mujer-web-01.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-49535 size-large\" src=\"https:\/\/42krunning.com\/wp-content\/uploads\/2022\/08\/camiseta-tecnica-42k-ozone-eclipse-mujer-web-01-1024x1024.jpg\" alt=\"La cintilla iliotibial es una estructura formada por tejido conectivo.\" width=\"1024\" height=\"1024\"\/><\/a><figcaption id=\"caption-attachment-49535\" class=\"wp-caption-text\">El dolor no aparece de forma inmediata, sino al cabo de unos minutos y va en aumento.<\/figcaption><\/figure>&#13;\n&#13;\n<p class=\"wp-block-paragraph\"> <\/p>&#13;\n<p>Sometimes this problem has been linked to weakness of the hip abductors, specifically the gluteus medius; it should be noted that in many cases this is true, but in many others, the cause-effect of the injury comes from another path.<\/p>&#13;\n&#13;\n<p class=\"wp-block-paragraph\">From our experience, we believe that a set of exercises alone does not solve the problem. <strong>First, the injury must be diagnosed<\/strong> as such, and <strong>then it must be evaluated what<\/strong> motor pattern or other reason <strong>is generating this recurrent friction<\/strong> and, with it, pain. Sometimes, a bone growth (epicondyle) on the lateral side of the femur can cause friction in the iliotibial band, greatly affecting both improvement and treatment, which may sometimes need to be surgical. <\/p>&#13;\n&#13;\n<p class=\"wp-block-paragraph\">At <strong>Chronic Clinical Specialties<\/strong>, we offer various types of treatments, including surgical options, guided by ultrasound.
